I guess from time to time a collector is lucky enough to stumble upon a box with a premium card in lots of the packs, a "hot box" so to speak. Are these done on purpose by the companies, or are they somehow done in error?

It's kind of a "sucker bet" for case breakers. They buy the case, break the boxes down until they hit the hot box. The remaining boxes go on Ebay with the tag that this one could be a hot box. Meanwhile the hot box is already being broken down and each card is being sold individually while the suckers are bidding on shadows."Chuckie doesn't take on 2-0.
