Indeed the first step toward finding out is to acknowledge you do not satisfactorily know already; so that no blight can so surely arrest all intellectual growth as the blight of cocksureness.--CS Peirce

Another recent case that comes to mind is Khalil Greene, whose career ended due to Social Anxiety Disorder.

Mike Pelfrey of the Mets has anxiety issues. Taylor Buchholz has severe depression.

the most famous is probably greinke. although he did quite ok the last years (but probably not as consistent as he could be)

Good point, but I really don't think it's a mechanical issue. You can see it in his face. He's trying too hard.

I forgot the most recent one in my memory.

AJ Burnett. He had the talent to be great but was a complete headcase in NY.

Rube Baker was a fictional catcher from the movie Major League II, and I think his throwing problems may have been based on Mackey Sasser's. Cowtipper only included him as a joke.

Greinke is another guy who's struggled with Social Anxiety Disorder. He's known to have an aversion to the spotlight.

Jim Eisenreich - Tourette's, not diagnosed as he had problems earlier in his career, and which cause him to miss multiple seasons, probably kept him from a very successful career. He was a good hitter.

Has anyone mentioned Steve Blass? Threw a WP to allow George Foster to score the winning run of the playoffs in 1972 and the next season could barely throw a strike (or something very close to that).
