Off the top of my head, I know it was done to Mel Ott, Bill Nicholson and Barry Bonds (by Arizona manager Buck Showalter in 2002, I think). It seems like there was one or two more. I have the list at home, in case someone else doesn't supply it here soon.Author of Big League Trivia - Facts, Figures, Oddities and Coincidences from our National Pastime.

Here's the list according to www.baseball-almanac.com
AL
Nap Lajoie Philadelphia 05-23-1901
NL
Del Bissonette Brooklyn 05-02-1928
Barry Bonds San Francisco 05-28-1998
Abner Dalrymple Chicago 08-02-1881
Bill Nicholson Chicago 07-23-1944 (Game 2)
Each instance is listed with more detail. It was Giant manager Mel Ott who ordered the walk to Bill Nicholson who had already hit 4 homers that day.
There is a descrepency on whether Ott was intentionally walked or not. Here's some info from http://www.baseballlibrary.com/ballp...ge=chronology:
Oct 5, 1929 - Mel Ott and Chuck Klein go into today's Giant-Phils doubleheader tied at 42 home runs apiece. In the opener‚ a 5-4 Phils victory‚ Ott manages a single‚ but Klein homers off Carl Hubbell in his first at bat to take the HR lead. In game 2‚ Ott singles in his first at bat. After that‚ manager Burt Shotton orders the Phillies pitchers‚ rather than give Ott a chance to tie Klein‚ to semi-intentionally walk him 5 times. The last (semi) intentional walk comes on a 3-2 count with the bases loaded as the Giants win‚ 12-3.
